What would be the best way to put a thumbnail image in an Excerpt?

In this othe blog i put them in using html and css in the title field, i want it to look similar to this, but this method is a little complicated.

Could i for example some how use the first image that appears in the post?

Any other ideas?

I already found a way to do this. I have added this tag before the title and styled it with css to float to the left.
